Abstract
An implantable medical device includes an activity sensor, a pulse generator, and a control module. The control module is configured to determine activity metrics from the activity signal and determine an activity metric value at a predetermined percentile of the activity metrics. The control module sets a lower pacing rate set point based on the activity metric value at the predetermined percentile.

An implantable cardiac device comprising:
- a housing configured to be implanted within a heart of a patient;
- an accelerometer positioned within the housing, the accelerometer configured to generate an accelerometer signal;
- and
- a control circuit configured to:
- determine, based on the accelerometer signal, a first plurality of activity counts over a first time period;
- determine a first number of activity counts of the first plurality of activity counts that are greater than a lower rate set point;
- compare the first number of activity counts to an expected number of activity counts;
- adjust the lower rate set point based on the comparison of the first number of activity counts to the expected number of activity counts;
- determine, based on the accelerometer signal, a second plurality of activity counts over a second time period, wherein the second time period is after the first time period;
- determine a pacing rate based on the second plurality of activity counts and the lower rate set point;
- and
- cause a pulse generator to generate pacing pulses at the pacing rate.
A method comprising:
- determining, by a control circuit of an implantable cardiac device based on an accelerometer signal, a first plurality of activity counts over a first time period, wherein the implantable cardiac device comprises:
- a housing configured to be implanted within a heart of a patient, and
- an accelerometer positioned within the housing configured to generate the accelerometer signal;
- determining, by the control circuit, a first number of activity counts of the first plurality of activity counts that are greater than a lower rate set point;
- comparing, by the control circuit, the first number of activity counts to an expected number of activity counts;
- adjusting, by the control circuit, the lower rate set point based on the comparison of the first number of activity counts to the expected number of activity counts;
- determining, by the control circuit based on the accelerometer signal, a second plurality of activity counts over a second time period, wherein the second time period is after the first time period;
- determining, by the control circuit, a pacing rate based on the second plurality of activity counts and the lower rate set point;
- and
- causing, by the control circuit, a pulse generator to generate pacing pulses at the pacing rate.
A non-transitory computer-readable medium storing instructions that, when executed by processing circuitry, cause the processing circuitry to:
- determine, based on an accelerometer signal of an implantable cardiac device, a first plurality of activity counts over a first time period, wherein the implantable cardiac device comprises:
- a housing configured to be implanted within a heart of a patient, and
- an accelerometer positioned within the housing configured to generate the accelerometer signal;
- determine a first number of activity counts of the first plurality of activity counts that are greater than a lower rate set point;
- compare the first number of activity counts to an expected number of activity counts;
- adjust the lower rate set point based on the comparison of the first number of activity counts to the expected number of activity counts;
- determine, based on the accelerometer signal, a second plurality of activity counts over a second time period, wherein the second time period is after the first time period;
- determine a pacing rate based on the second plurality of activity counts and the lower rate set point;
- and
- cause a pulse generator to generate pacing pulses at the pacing rate.
