Abstract
A method and apparatus are disclosed for puncturing and using the puncturing device to evaluate and identify the anatomical space that the puncturing device has entered. The physician can then determine if the puncturing device has entered the desired anatomical space or an unintended anatomical space.
Claims20
An RF guidewire for creating a transseptal puncture, comprising:
- an electrically conductive body extending from a proximal end to a distal end;
- an electrically insulative covering disposed over the electrically conductive body along a majority of a length thereof, the electrically insulative covering defining at least one exposed distal electrode at the distal end,
- wherein the exposed distal electrode is limited in surface area relative to the total length of the electrically conductive body;
- and
- wherein the at least one distal electrode is electrically continuous along its length and is configured to be electrically connected to both an RF electrosurgical generator for delivering electrical energy for puncturing and an electroanatomic mapping (EAM) system.

A method of creating an atrial septal defect, the method comprising:
- connecting an RF guidewire to an RF electrosurgical generator and to an electroanatomic mapping (EAM) system;
- positioning a distal electrode of the RF guidewire against an interatrial septum via an intravascular sheath or dilator;
- applying RF energy from the RF electrosurgical generator through the RF guidewire to puncture the interatrial septum;
- advancing the distal electrode of the RF guidewire through the puncture and into an anatomical space on the other side of the septum;
- and
- evaluating a characteristic of the anatomical space via the EAM system using the distal electrode.

A system for creating transseptal access, comprising:
- an RF guidewire comprising an electrically conductive body, an electrically insulative covering disposed over a majority of the electrically conductive body, and an exposed distal electrode at a distal end of the electrically conductive body, wherein the exposed distal electrode is electrically continuous along its length;
- an RF electrosurgical generator electrically connectable to the RF guidewire for delivering RF energy through the distal electrode to puncture tissue;
- and
- an electroanatomic mapping (EAM) system electrically connectable to the RF guidewire for mapping a position of the distal electrode within a patient's anatomy,
- wherein the RF guidewire is configured to serve as a delivery electrode for the RF electrosurgical generator and as a mapping electrode for the EAM system.
