A bone anchor assembly includes a screw shank with capture portion having a partial spherical shape and an internal drive socket, and an anchor portion configured for fixation to the bone. The assembly also includes a receiver with a channel for receiving a rod and an internal cavity adjacent a bottom opening for receiving the capture portion of the screw shank. The assembly further includes a separate element insertable within the receiver and configured to compressively engage with the capture portion of the screw shank so as to lock the bone anchor assembly with a locking mechanism in a locked configuration. An annular planar top surface on the capture portion of the screw shank is configured to remain spaced apart from the separate element so as to not directly receive a downwardly-directed force from the locking mechanism to result in the locked configuration.
