A two-piece wearable cardiac monitoring device for monitoring a cardiac condition of a patient and optionally providing therapy includes a garment, ECG sensing electrodes, ECG sensor circuitry integrated into the garment, conductive wiring woven into the garment, and at least one separable therapy delivery component. The separable therapy component(s) includes one or more energy storage modules and a therapy control module. The cardiac monitoring device also includes an electrical connector, disposed in the garment and configured to facilitate electrical coupling of the separable therapy delivery component(s), and at least one processor. The processor(s) is configured to, on detecting separation of the therapy delivery component(s), cause the cardiac monitoring device to automatically operate as a monitoring device and, on detecting installation of the separable therapy delivery component(s), cause the cardiac monitoring device to operate as a treatment device by further providing therapeutic shocks to the patient in response to an arrhythmia.
