A radiotherapy couch top configured to support a subject thereon can include a first lateral width in a first longitudinal region of the radiotherapy couch top and a second lateral width in a second longitudinal region of the radiotherapy couch top, where the second lateral width is smaller than the first lateral width. The radiotherapy couch top can further include a curved lower surface. The curved lower surface can be configured to help reduce imaging artifacts during radiotherapy procedures, particularly for kV imaging beam angles that are parallel or close to parallel to the couch top surfaces. The narrower second longitudinal region can help enable direct irradiation of a subject's head from a greater range of treatment angles without the radiotherapy beam passing through the couch top material, improving treatment accuracy and versatility.
