A method comprising:

• receiving, by a wireless device, an uplink grant via a monitoring occasion associated with a synchronization signal and physical broadcast channel block (SSB), wherein the SSB is determined by a most recent of:
• an activation command of a transmission configuration indicator (TCI) state;
• a random access procedure;
• or
• a configured-grant based uplink transmission while the wireless device is in a radio resource control (RRC) inactive state;
• and
• transmitting an uplink transmission based on the uplink grant.

A non-transitory computer-readable medium comprising instructions that, when executed by one or more processors of a wireless device, cause the wireless device to:

• receive an uplink grant via a monitoring occasion associated with a synchronization signal and physical broadcast channel block (SSB), wherein the SSB is determined by a most recent of:
• an activation command of a transmission configuration indicator (TCI) state;
• a random access procedure;
• or
• a configured-grant based uplink transmission in a radio resource control (RRC) inactive state;
• and
• transmit an uplink transmission based on the uplink grant.
