Abstract
Devices and methods of endolumenal formation of gastric sleeves are described. Some embodiments allow templating of a gastric sleeve by a gastric bougie, exposing a selected amount of tissue for suturing access, while maintaining sufficient internal working space for suturing within the template lumen.
Claims30
A bougie for shaping a portion of a stomach to define suturing locations for formation of a gastric passageway for food, the bougie comprising:
- (a) a wall defining a longitudinal lumen, and having an exterior surface sized and shaped to be insertable to the stomach in a position defining the gastric passageway for food along an extent and around a circumference of the bougie;
- (b) a plurality of spaced wall fenestrations longitudinally distributed along the wall and along the extent of the bougie defining the gastric passageway, and sized to receive and position a predetermined thickness of gastric tissue for suturing by a needle passing within the lumen and radially near the wall;
- and
- wherein a geometry of at least some of said fenestrations is modifiable while said bougie is positioned in said stomach for formation of the gastric passageway for food.

A method of intralumenally suturing a gastric sleeve, comprising:
- inserting into a stomach a bougie having a wall comprising a plurality of longitudinally spaced cutaway regions comprising separated fenestrations;
- drawing vacuum through said bougie to define a gastric passageway for food, such that stomach tissue from opposing sides of the stomach is drawn to the bougie wall, wrapping around it, and partially intruding into said fenestrations, one such side in each separated portion of the cutaway regions;
- and
- suturing the drawn tissue from two sides to each other by a needle passing within the lumen and radially near the wall while the bougie remains in place with the gastric passageway defined until the suturing is completed;
- wherein said fenestrations are sized to receive and position a predetermined thickness of gastric tissue for said suturing;
- and
- wherein a geometry of said fenestrations is modified before, during or after said suturing.
