Apparatus and method for self-reporting medical information

A system comprising:

• a database configured to store at least one patient record and an initial performance assessment related to the at least one patient record;
• an interface having an output and an input, the interface being configured to receive self-reported medical data from at least one medical service provider via a communication link;
• a self-reporting module coupled to the interface and the database, the self-reporting module configured to receive the at least one patient record from the data base and determine at least one inquiry for a medical provider at the output of the interface based upon analyzing the at least one patient record;
• a performance evaluation module coupled to the database, the interface, and the self-reporting module, the performance evaluation module configured to re-calculate the initial assessment and forming an updated assessment based upon the self-reported medical data received at the input of the interface, the self-reported medical data being received from the provider in response to the at last one inquiry.
