Electronic healthcare form generation with bundled supplemental documentation

A system comprising:

• a first device including a hardware processor and a user interface;
• and
• a second device including a hardware processor, a user interface, and a camera;
• the system being configured to perform operations comprising:
• obtaining, from a user, a plurality of values of fields of a healthcare form, including obtaining at least a subset of the plurality of values via the user interface of the first device operated by the user;
• transferring a workflow for preparing a form bundle from the user interface of the first device to the user interface of the second device;
• in response to transferring the workload, capturing a live image comprising supplemental documentation for the healthcare form, from the camera of the second device operated by the user;
• generating, by the second device, the form bundle comprising (a) an electronic version of the healthcare form populated with the plurality of values, and (b) the image comprising the supplemental documentation for the healthcare form;
• and
• responsive to user input to the user interface of the second device, transmitting the form bundle from the second device to a healthcare organization.

A method comprising:

• obtaining, from a user, a plurality of values of fields of a healthcare form, including obtaining at least a subset of the plurality of values via a user interface of a first device operated by the user;
• transferring a workflow for preparing a form bundle from the user interface of the first device to a user interface of a second device operated by the user;
• in response to transferring the workload, capturing a live image comprising supplemental documentation for the healthcare form, from a camera of the second device operated by the user;
• generating, by the second device, the form bundle comprising (a) an electronic version of the healthcare form populated with the plurality of values, and (b) the image comprising the supplemental documentation for the healthcare form;
• and
• responsive to user input to the user interface of the second device, transmitting the form bundle from the second device to a healthcare organization.
