## Abstract

A satiation device is described which includes a sheath or liner extending from the proximal or middle stomach to the distal antrum. Food ingested by the patient passes through the sheath or liner, thereby minimizing contact between the ingested food and the stomach. It is believed that over time, reduced contact between food and the stomach will result in decreased Ghrelin production by the patient and a consequent decrease in appetite. In some embodiments, the satiation device may also include a proximal pouch and/or a distal bypass tube.

## Claims

### Claim 1

An implant for a patient having an esophagus, a gastro-esophageal junction region, and a small intestine, comprising:
a gastro-esophageal sleeve extending from an upstream end to a downstream end, the upstream end of the sleeve being configured to be attached to the gastro-esophageal junction region such that food from the esophagus enters the upstream end of the sleeve, wherein the upstream end of the gastro-esophageal sleeve comprises a gastro-esophageal junction tissue-contacting surface; and
an intestinal tube having a proximal portion and a distal portion, wherein the proximal portion is fluidly coupled to the downstream end of the sleeve, wherein a space between a distal end of the proximal portion and a proximal end of the distal portion does not include a tube portion and is exposed to a portion of an intestinal wall,
wherein the gastro-esophageal sleeve is funnel-shaped, and
wherein the gastro-esophageal sleeve is self-biased towards an open arrangement.

### Claim 2 (depends on 1)

The implant of claim 1, wherein, when the sleeve is secured within the gastro-esophageal junction region, the space is configured to align with an ampulla of Vater.

### Claim 3 (depends on 1)

The implant of claim 1, wherein the proximal and distal portions of the tube are connected by a connection element extending therebetween.

### Claim 4 (depends on 3)

The implant of claim 3, wherein the connection element is a tether.

### Claim 5 (depends on 1)

The implant of claim 1, wherein the sleeve includes a plurality of attachment points, and wherein the sleeve is configured to be secured within the gastro-esophageal junction region at the attachment points.

### Claim 6 (depends on 5)

The implant of claim 5, wherein the sleeve is configured to be secured to the gastro-esophageal region using sutures, and the plurality of attachment points include holes adapted to pass the sutures therethrough.

### Claim 7 (depends on 1)

The implant of claim 1, further including one or more attachment elements configured to secure the implant to tissue within the gastro-esophageal junction region.

### Claim 8 (depends on 7)

The implant of claim 7, wherein the one or more attachment elements include one of sutures, clips, or stents.

### Claim 9 (depends on 1)

The implant of claim 1, wherein the proximal end of the distal portion of the tube has a larger diameter relative to a diameter of the distal end of the proximal portion of the tube.

### Claim 10

An implant for a patient having an esophagus, a gastro-esophageal junction region, and a small intestine, comprising:
a gastro-esophageal sleeve extending from an upstream end to a downstream end, the upstream end of the sleeve being configured to be attached to the gastro-esophageal junction region such that food from the esophagus enters the upstream end of the sleeve, wherein the upstream end of the gastro-esophageal sleeve comprises a gastro-esophageal junction tissue-contacting surface; and
an intestinal tube having a proximal portion and a distal portion, wherein the proximal portion is fluidly coupled to the downstream end of the sleeve such that food that enters the sleeve passes into the proximal portion of the tube, and wherein the tube is configured so that food that exits the proximal portion is exposed to a portion of an intestinal wall prior to entering the distal portion; and
one or more attachment elements configured to attach the upstream end of the sleeve to the gastro-esophageal junction region,
wherein the gastro-esophageal sleeve is funnel-shaped, and
wherein the gastro-esophageal sleeve is self-biased towards an open arrangement.

### Claim 11 (depends on 10)

The implant of claim 10, wherein, when the sleeve is attached to the gastro-esophageal junction region, the portion of the intestinal wall includes an ampulla of Vater.

### Claim 12 (depends on 10)

The implant of claim 10, wherein the proximal and distal portions of the tube are connected by a connection element extending therebetween.

### Claim 13 (depends on 10)

The implant of claim 10, wherein the distal portion of the tube includes a funnel-shaped proximal portion.

### Claim 14 (depends on 10)

The implant of claim 10, wherein the upstream end of the sleeve includes a plurality of attachment points, and wherein the sleeve is configured to be attached to tissue in the gastro-esophageal junction region using one or more attachment elements passing through the plurality of attachment points.

### Claim 15 (depends on 14)

The implant of claim 14, wherein the plurality of attachment points includes a plurality of holes.
