## Abstract

A material fixation system is particularly adapted to improve the tendon-to-bone fixation of hamstring autografts, as well as other soft tissue ACL reconstruction techniques. The system is easy to use, requires no additional accessories, uses only a single drill hole, and can be implanted by one person. Additionally, it replicates the native ACL by compressing the tendons against the aperture of the tibial tunnel, which leads to a shorter graft and increased graft stiffness. It is adapted to accommodate single or double tendon bundle autografts or allografts. It also provides pull out strength measured to be greater than 1000 N.

## Claims

### Claim 1

A method of making an orthopedic repair by fixing a soft tissue graft to bone, comprising:
creating a tunnel within a desired bone site, said tunnel extending through a first bone member and comprising a blind hole in a second bone member;
placing a soft tissue graft on an implant;
securing said implant within said blind hole, such that a plurality of ends of said soft tissue graft extend from said implant and substantially entirely through the tunnel in said first bone member;
sliding a second implant, comprising a plurality of sheath members connected together at a distal end thereof so that the sheath members are expandable outwardly about a hinge and a cortical hook on a proximal end of one of said sheath members, along said soft tissue graft ends, with the sheath members being disposed between the soft tissue graft ends, into said tunnel in the first bone member, to a predetermined location wherein said cortical hook engages a cortical surface of said tunnel on said first bone member;
tensioning said soft tissue graft ends; and
expanding said second implant outwardly to compress the soft tissue graft ends against a wall defining said bone tunnel; wherein one of said sheath members comprises a snap post and another of said sheath members comprises a snap hole, said snap post and said snap hole being engaged with one another when said sheath members are in an initial unexpanded orientation, wherein the expanding step is initiated by disengaging the snap post from the snap hole.

### Claim 2 (depends on 1)

The method as recited in claim 1, wherein the tensioning step is performed by pulling the soft tissue graft ends taut, to a desired level of tension.

### Claim 3 (depends on 2)

The method as recited in claim 2, wherein the pulling step is performed using manual traction.

### Claim 4 (depends on 1)

The method as recited in claim 1, wherein the expanding step comprises inserting a screw distally between said sheath members to pivot said sheath members outwardly about said hinge.

### Claim 5 (depends on 4)

The method as recited in claim 4, wherein the screw inserting step comprises rotating said screw, using a driver, until the screw has moved distally sufficiently so that it is fully seated within said sheath members, said screw being fully seated when a bullnose screw head on a proximal end of the screw is flush with the cortical surface of said tunnel.

### Claim 6 (depends on 1)

The method as recited in claim 1, wherein a distal end of said second implant is in close proximity to a distal end of the tunnel in said first bone member when the second implant is fully inserted and deployed.

### Claim 7 (depends on 1)

The method as recited in claim 1, wherein said plurality of sheath members comprises two sheath members.

### Claim 8 (depends on 4)

The method as recited in claim 4, wherein said screw is a tapered screw.

### Claim 9 (depends on 1)

The method as recited in claim 1, wherein said hinge comprises a hinge protrusion disposed on a first of said sheath members and a hinge slot disposed on a second of said sheath members, wherein said hinge protrusion and said hinge slot engage one another.

### Claim 10 (depends on 9)

The method as recited in claim 9, wherein said second implant further comprises a second hinge protrusion disposed on said second sheath member and a second hinge slot on said first sheath member, wherein said second hinge protrusion and said second hinge slot engage one another.

### Claim 11 (depends on 4)

The method as recited in claim 4, wherein said screw comprises external threads and said sheath members comprise complementary internal threads, so that the screw is advanced distally by engagement of said external and internal threads.

### Claim 12

A method of making an orthopedic repair by fixing a soft tissue graft to bone, comprising:
creating a tunnel within a desired bone site having a proximal opening and a distal end;
anchoring a soft tissue graft at a distal end of the tunnel on a first implant, so that a plurality of ends of said soft tissue graft extend proximally through the tunnel and out of said proximal opening;
inserting a second implant comprising a plurality of members which are expandable outwardly between said soft tissue graft ends;
sliding said second implant into said tunnel to a predetermined location; and
expanding said second implant outwardly to compress the soft tissue graft ends against a bony wall defining said tunnel by inserting a screw distally between said sheath members to expand said sheath members outwardly, wherein the screw inserting step comprises rotating said screw, using a driver, until the screw has moved distally sufficiently so that it is fully seated within said sheath members, said screw being fully seated when a screw head on a proximal end of the screw is substantially flush with the cortical surface of said tunnel wherein one of said sheath members comprises a snap post and another of said sheath members comprises a snap hole, said snap post and said snap hole being engaged with one another when said sheath members are in an initial unexpanded orientation, wherein the expanding step is initiated by disengaging the snap post from the snap hole.

### Claim 13 (depends on 12)

The method as recited in claim 12, wherein a distal end of said second implant is in close proximity to a distal end of the tunnel in said first bone member when the second implant is fully inserted and deployed.

### Claim 14 (depends on 12)

The method as recited in claim 12, wherein said plurality of expandable members comprises a plurality of sheath members connected together at a distal end thereof, so that the sheath members are expandable outwardly about said hinge.

### Claim 15 (depends on 12)

The method as recited in claim 12, wherein said screw is a tapered screw.

### Claim 16 (depends on 14)

The method as recited in claim 14, wherein said hinge comprises a hinge protrusion disposed on a first of said sheath members and a hinge slot disposed on a second of said sheath members, wherein said hinge protrusion and said hinge slot engage one another.

### Claim 17 (depends on 16)

The method as recited in claim 16, wherein said second implant further comprises a second hinge protrusion disposed on said second sheath member and a second hinge slot on said first sheath member, wherein said second hinge protrusion and said second hinge slot engage one another.

### Claim 18 (depends on 14)

The method as recited in claim 14, wherein said screw comprises external threads and said sheath members comprise complementary internal threads, so that the screw is advanced distally by engagement of said external and internal threads.
