Abstract
Certain substituted urea derivatives selectively modulate the cardiac sarcomere, for example by potentiating cardiac myosin, and are useful in the treatment of systolic heart failure including congestive heart failure.

A compound of the formula
- or a pharmaceutically acceptable salt thereof,
- where one of Z1 and Z2 is —NR15C(O)NR16R4 and the other of Z1 and Z2 is R3;
- R4 is optionally substituted heteroaryl;
- R3 is chosen from hydrogen, halo, cyano, hydroxyl, optionally substituted alkyl, optionally substituted heterocycloalkyl, and optionally substituted heteroaryl;
- R1 and R2 are independently chosen from hydrogen, halo, cyano, optionally substituted alkyl, optionally substituted heterocycloalkyl, and optionally substituted heteroaryl;
- R15 and R16 are independently chosen from hydrogen, and optionally substituted alkyl;
- R5 is optionally substituted piperazinyl;
- and
- L is lower alkylene.
