Abstract
An exemplary method includes detecting a QRS complex using cutaneous electrodes, during the QRS complex, detecting an R-wave of a ventricle using an intracardiac electrode, determining if the R-wave occurred during a first, predetermined percentage of the QRS complex width and, based at least in part on the determining, deciding whether a patient is likely to respond to cardiac resynchronization therapy. Such a method may set the predetermined percentage to approximately 50%. An exemplary model includes a parameter for a percentage for the timing of an EGM R-wave with respect to the total width of an ECG QRS complex. Various other exemplary methods, devices, systems, etc. are also disclosed.
Claims20
A method of assessing synchrony of a ventricular contraction, said method comprising:
- acquiring a first type of cardiac electrogram comprising QRS complex information using a first set of electrodes while also acquiring a second type of cardiac electrogram comprising R-wave information using a second set of electrodes different from the first set of electrodes, wherein the first type of cardiac electrogram and the second type of cardiac electrogram comprise the same ventricular contraction;
- determining a width of a QRS complex from the QRS complex information contained in the first type of cardiac electrogram;
- determining a time of occurrence of an R-wave from the R-wave information contained in the second type of cardiac electrogram;
- determining a time from a beginning of the QRS complex to the occurrence of the R-wave;
- processing the width of the QRS complex together with the time to obtain a measure;
- and
- comparing the measure to a threshold to classify the ventricular contraction as synchronous or dyssynchronous.
A system comprising:
- a first device configured to acquire a first type of cardiac electrogram comprising QRS complex information using a first set of electrodes;
- a second device configured to acquire a second type of cardiac electrogram comprising R-wave information using a second set of electrodes different from the first set of electrodes, wherein the first type of cardiac electrogram and the second type of cardiac electrogram comprise the same ventricular contraction;
- a computing device configured to receive the first type of cardiac electrogram from the first device and the second type of cardiac electrogram from the second device, wherein the computing device comprises control logic configured to:
- determine a width of a QRS complex from the QRS complex information contained in the first type of cardiac electrogram;
- determine a time of occurrence of an R-wave from the R-wave information contained in the second type of cardiac electrogram;
- determine a time from a beginning of the QRS complex to the occurrence of the R-wave;
- process the width of the QRS complex together with the time to obtain a measure;
- and
- compare the measure to a threshold to classify the ventricular contraction as synchronous or dyssynchronous.
