## Abstract

Focal and segmental glomerulosclerosis (FSGS) is a kidney disorder of unknown etiology and up to 20% of patients on dialysis have this diagnosis. A large family with hereditary FSGS carries a missense mutation in the TRPC6 gene on chromosome 11q, encoding the ion channel protein Transient Receptor Potential Cation Channel 6. The missense mutation is a P112Q substitution, which occurs in a highly conserved region of the protein, enhances TRPC6-mediated calcium signals in response to agonists such as angiotensin II, and alters the intracellular distribution of TRPC6 protein. Previous work has emphasized the importance of cytoskeletal and structural proteins in proteinuric kidney diseases. Our findings suggest a novel mechanism for glomerular disease pathogenesis.

## Claims

### Claim 1

A method of identifying a subject at increased risk of developing Focal and Segmental Glomerulosclerosis (FSGS), comprising:
analyzing the nucleotide sequence of a Transient Receptor Potential Cation Channel, Subfamily C, Member 6 (TRPC6) gene in a nucleic acid sample obtained from the subject;
comparing the nucleotide sequence of the sample gene to the nucleotide sequence of a reference wild-type TRPC6 gene comprising SEQ ID NO: 64 to determine one or more differences between the nucleotide sequence of the sample gene and the reference gene; and
identifying the subject as being at increased risk of developing FSGS if the sequence of the gene of the subject differs from the reference gene at position 335 of the coding sequence of the reference gene, or identifying the subject as being at no increased risk if the sequence of the gene of the subject does not differ from the reference gene at position 335 of the coding sequence.

### Claim 2 (depends on 1)

The method of claim 1 wherein the sequence difference of the sample TRPC6 gene is a non-synonymous difference with respect to the reference wild-type TRPC6 gene.

### Claim 3 (depends on 1)

The method of claim 1 wherein the sequence difference is an adenine at nucleotide 335.

### Claim 4 (depends on 1)

The method of claim 1 wherein the sequence difference is in a codon which encodes glutamine at residue 112 of a TRPC6 protein comprising SEQ ID NO: 63 or 65.

### Claim 5 (depends on 1)

The method of claim 1 wherein oligonucleotide probes are used to detect the sequence difference.

### Claim 6 (depends on 1)

The method of claim 1 wherein an amplification reaction is used to determine the sequence difference.

A method of identifying a subject with Focal and Segmental Glomerulosclerosis (FSGS), comprising:
analyzing the nucleotide sequence of a Transient Receptor Potential Cation Channel Subfamily C, Member 6 (TRCP6) gene in a nucleic acid sample obtained from the subject;
comparing the nucleotide sequence of the sample gene to the nucleotide sequence of a reference wild-type-TRPC6 gene comprising SEQ ID NO: 64 to determine one or more differences between the nucleotide sequence of the sample gene and the reference gene; and
identifying the subject as having FSGS if the sequence of the gene of the subject differs from the reference gene at position 335 of the coding sequence of the reference gene, or identifying the subject as not having FSGS if the sequence of the gene of the subject does not differ from the reference gene at position 335 of the coding sequence.
