Abstract
Guidewire-deliverable devices and methods of using the devices for separating adjacent layers of tissue and for delivering a surgical prosthesis to an operating site. A guidewire-delverable cannula provides an elongate window for accessing the operating site with a tissue-separating device. The tissue-separating device may include one of several types of an inflatable balloon or a tissue-dissecting member. The tissue-separating balloon may simultaneously deliver the prosthesis during tissue separation and may further be detached to remain with the prosthesis over the operating site. Alternatively, the prosthesis may be separately provided after the adjacent layers of tissue have been separated.
Claims20
An surgical mesh assembly for use in an operating space between adjacent layers of tissue comprising:
- a. a surgical mesh comprising:
- a first major surface and
- a second major surface in facing opposition to, and peripherally contiguous with said first major surface to define a fluid tight interior space therebetween;
- and
- b. a fluid entry port in fluid communication with said fluid tight interior space through one of said major surfaces and disconnectably connected to a fluid delivery source whereby said surgical mesh may be deployed over an operating site by delivering a fluid through said fluid entry port into said interior space so as to expand said surgical mesh from a contracted configuration to an expanded configuration;
- wherein at least one of said major surfaces is a porous mesh having a biodegradable filling material extending into pores of said porous mesh to maintain the fluid integrity of said fluid tight interior space, whereby decomposition of said biodegradable filling material exposes the pores of said porous mesh to permit tissue ingrowth therethrough.
A balloon-supported surgical mesh assembly for use in an operating spa between adjacent layers of tissue comprising:
- a. a fluid entry port disconnectably connected to a fluid delivery source;
- b. a hollow expandable balloon frame having a first elongate tubular balloon segment having one end in fluid communication with a first end of a second elongate tubular balloon segment, said second balloon segment having an opposed second end in fluid communication with a first end of a third elongate tubular balloon segment, said third balloon segment having an opposed second end in fluid communication with a first end of a fourth elongate tubular balloon. segment, said fourth balloon segment having an opposed second end in fluid communication with the opposed end of said first elongate tubular balloon segment;
- wherein said elongate balloon segments are hollow tubular segments having a fluid tight interior space in fluid communication with said fluid entry port;
- and
- c. a porous surgical mesh securably attached to a pair of opposing balloon segments and having pores to permit tissue ingrowth therethrough whereby said porous surgical mesh may be deployed over an operating site by delivering a fluid through said entry port into said interior space so as to expand said balloon frame from a contracted configuration to an expanded configuration.
