Abstract
Surgery is performed with a pulsed heat-producing device that selectively heats a region in a specific tissue within a patient destroying the tissue. The pulsed heat-producing device may be a coherent optical source that is guided by laser fiber to the tissue to be destroyed. In another embodiment, the pulsed heat-producing device is a focussed ultrasound transducer which concentrates ultrasonic energy at a focal point within the specific tissue. A magnetic resonance imaging system employing a real-time temperature-sensitive pulse sequence monitors the heated region of the tissue to provide temperature profiles allowing an operator to alter the position and size of the heated region.
Claims7
A magnetic resonance (MR) pulsed heat system for allowing an operator to selectively heat tissue within a patient comprising:
- a) pulsed heat-producing means adapted for concentrating energy at an application point;
- b) positioning means for positioning the application point of the pulsed heat-producing means in a specific tissue within the patient so as to create a heated region within the specified tissue;
- c) operator responsive control means for enabling said operator to control the positioning means;
- d) an MR imaging means for providing real-time temperature-sensitive MR images of the heated region comprising:
- 1. a radiofrequency (RF) transmitter for transmitting with a repetition time T.sub.R a plurality of RF excitation pulses into said patient;
- 2. a gradient means for:
- i. producing a plurality of time-varying magnetic field gradients in an "X" direction and a plurality of time-varying magnetic field gradients in a "Y" direction orthogonal to the "X" direction, both gradients applied simultaneously with each RF excitation pulse thereby exciting transverse magnetization of an elongated excitation region in said patient,
- ii. applying with repetition time T.sub.R a plurality of dephasing magnetic field gradients of a predetermined duration having a predetermined amplitude of a first polarity, and
- iii. applying with repetition time T.sub.R a plurality of readout magnetic field gradients each having a second polarity opposite the first polarity, and having a product of duration multiplied by amplitude substantially larger than a product of the amplitude and duration of the dephasing magnetic field gradient;
- 3. receiver means for receiving with repetition time T.sub.R a plurality of MR response signals from the elongated excitation region simultaneously with the application of each readout radiant;
- and
- 4. computation means for computing a temperature-sensitive profiles from each MR response signal from the receiver means;
- and
- e) display means for displaying the temperature-sensitive profiles to said operator.
The MR pulsed heat system system of claim 2 wherein the pulsed heat-producing means comprises:
- a) a source of pulsed optical energy;
- b) an invasive device adapted for insertion into the patient to reach the specific tissue of the patient;
- and
- c) an optical fiber having an external end and an internal end adapted to be fitted into the invasive device with its internal end within the specific tissue, the fiber being adapted to pass the optical energy from the pulsed optical energy source into the external end, to its application point in the specific tissue adjacent the internal end.
A method of performing heat surgery on a patient as guided by magnetic resonance (MR) imaging comprising the steps of:
- a) creating an internal image of tissues of said patient;
- b) determining the position of a specified tissue in the patient;
- c) applying pulsed heat at a predetermined pulse frequency to the specified tissue to create a heated region within the specified tissue;
- d) monitoring the heated region with an MR imaging device by:
- 1. transmitting an RF excitation pulse into said patient;
- 2. producing a first time-varying magnetic field gradient in an "X" direction and a second time-varying magnetic field gradient in a "Y" direction orthogonal to the "X" direction, both gradients applied simultaneously with the RF excitation pulse so as to excite transverse magnetization in an elongated excitation region in said patient;
- 3. applying a dephasing magnetic field gradient to said patient for a predetermined duration having a predetermined amplitude of a first polarity, and
- 4. applying a readout magnetic field gradient in a "Z" direction along the length of the elongated excitation region, having a second polarity opposite the first polarity, and having a product of duration multiplied by amplitude substantially larger than a product of duration multiplied by amplitude of the dephasing magnetic field gradient;
- 5. receiving simultaneously with the application of the readout gradient, an MR response signal M.sub.sig which is equal to longitudinal magnetization M.sub.Z, defined by:
- ##EQU2## from the elongated excitation region, where M.sub.0 is the equilibrium magnetization, T.sub.1 is the spin lattice relaxation time, and T.sub.R is the pulse-sequence repetition time;
- 6.
