Syn-isomer of 3,7-disubstituted-3-cephem-4-carboxylic acid compounds of the formula:

##STR34## wherein R.sup.1 is amino or formamido,
R.sup.2 is allyl,
R.sup.3 is carboxy, and
R.sup.4 is 1-allyl-1H-tetrazol-5-yl, and pharmaceutically acceptable salts thereof.
