A thermal transfer pad that can be cooled or heated and applied to the skin, with a profiled (non-flat) surface that reduces the rate of heat transfer between skin tissue and the pad to better regulate skin temperature. The pad may be applied and left on, compared to existing pads that should be periodically removed and then reapplied. The profiled surface may have protrusions of various patterns, including ridges, pillars, rings, or honeycombs; important parameters that affect performance are the amount of effective transfer surface contacting the skin, and the height of the protrusions. Illustrative ranges of these parameters that may be effective in one or more embodiments of the invention are protrusion heights between 1 mm and 6 mm, and a ratio of effective transfer surface to total application surface between 5% and 40%.
