A bone anchor comprising an enlarged head and an elongate bone screw body having a shape conducive for linear impacted introduction into bone. Helical flanges extend about the body of the bone anchor and are interrupted by a plurality of elongate axial grooves. The helical flanges can be aligned or misaligned. The bone anchors can be either cannulated bone anchors or solid anchors absent of cannulation. The bone anchors can be utilized in a variety of applications such as to secure an intervertebral spacer, a bone plate, or for fixation across bone segments such as when inducing fusion at an SI joint. The bone anchor can include an elongate window for the introduction of bone material to enhance fusion. In one method, the bone anchor is impacted into the bone and then rotated to continue advancement of the bone anchor into the bone.
