A computer-implemented cognitive health assessment platform comprising:

• a multi-modal engine utilizing artificial intelligence and accepting real-time patient cognitive health related data;
• a digital twin framework accepting data from said multi-modal engine;
• a real-time streaming analytics engine that accepts data from said digital twin framework;
• and
• an avatar interface integration subsystem comprising real-time conversational AI and providing real-time patient cognitive health related data to said multi-modal engine, wherein said platform generates and reports substantially real time cognitive assessments.

A method for generating cognitive health assessment comprising:

• generating cognitive real-time health related data from patient wearable sensors and from avatar interface integration subsystem to generate substantially real-time multi-mode data;
• utilizing said multi-mode data to generate a cognitive digital twin of the patient;
• utilizing said multi-mode data for clinical trial matching;
• and
• generating a substantially real-time cognitive index based on said multi-mode data, said cognitive index used in generating substantially real-time cognitive assessments.
