Techniques to determine a physical state of a patient and, based on the physical state, dynamically update one or more parameters that define operation of a medical device, such as, one or more stimulation parameters, one or more sense parameters, and/ or signal processing parameters (e.g., artifact cancellation parameters). These parameters may be utilized by the medical device to sense physiological signals from a patient and/or deliver electrical stimulation to target tissue of the patient. In some examples the medical device may include an implantable medical device (IMD). The physical state may include one or more of a posture state, an activity state, and in some examples, a degree of distraction of the patient. Electrode position relative to target tissue may change with physical state.
