The present disclosure relates to a novel salt and salt forms of Compound A. The disclosure is also directed to pharmaceutical compositions containing at Compound A citrate salt or salt forms and to the therapeutic use of such salts, salt forms, and compositions thereof.

A method of treating a leukemia in a human in need thereof comprising orally administering a citrate salt of Compound A:
- wherein Compound A is administered at a dosage of about 270 mg twice daily, further wherein Compound A is administered without a CYP3A4 inhibitor.
A method of treating a leukemia in a human in need thereof, comprising orally administering a citrate salt of Compound A:
- wherein the human weighs less than 40 kg and Compound A is administered at a dosage of about 160 mg/m2 twice daily, further wherein Compound A is administered without a CYP3A4 inhibitor.
