A medicament delivery system for delivering therapeutic agents into a gingival crevice proximate to a junctional epithelium. A customized orthodontic device may be configured to conform to a dental anatomy of a patient and to carry a medicament such that, when the customized orthodontic device is seated on one or more teeth, the medicament is moved into a selected gingival crevice. The customized orthodontic device may be configured as a full-arch retainer, a partial-arch retainer, or a single-tooth cap, and may include a reservoir defined between walls for controlled medicament release. In embodiments, a targeting retainer may include one or more targeting holes aligned with selected gingival crevices, and a cartridge having a nozzle, a reservoir, and a piston may deliver the medicament through a selected targeting hole into the selected gingival crevice. The medicament may thereafter be absorbed through the junctional epithelium.
