A simulated weight-bearing positioning apparatus includes a base configured to support at least a portion of a patient's leg and a footpad movable relative to the base between a stowed position and a deployed position. The footpad includes a patient-contacting surface configured to contact a plantar surface of the patient's foot. One or more guide features guide movement of the footpad relative to the base. The footpad may translate relative to the base, rotate relative to the base, and engage the base at a selected deployed angle. A locking feature retains the footpad in the deployed position so that the patient's foot may be maintained in a simulated weight-bearing position for imaging, surgery, or another clinical procedure. In some embodiments, the apparatus permits repeated deployment and re-stowing of the footpad during a procedure.
