A gas lift valve includes a housing with an interior passage and first and second ports. The first port is exposed to either annulus pressure or tubing pressure, while the second port is exposed to the other. The interior passage has upper and lower shoulders on both sides of the first port. A pressure-sensitive piston is movably positioned in the housing, capable of axial movement in response to a pressure differential between the interior passage and a dome pressure. An indexing assembly includes a rotatable collar within the interior passage, between the upper and lower shoulders. The collar has an orifice port, an internal profile, and upper and lower edges. A biasing element pushes the collar toward the lower shoulder. A collet on the piston engages the collar's profile and moves the collar, which then rotates incrementally when the edges contact the respective shoulders.
