Passive flashback devices enable one-handed guidewire insertion into a patient. A first device includes an elongate housing with a handle, a guidewire placement assembly with a wire feed channel, a passive guidewire feed surface, and a sheath brake, with or without an elastomeric microvalve assembly in the wire feed channel. A second device adds a guidewire carriage that slides along the housing between open and closed positions, with the microvalve assembly situated in a channel through a stanchion of the guidewire placement assembly. In both devices the guidewire moves freely and independently through the device without generating a vacuum. The microvalve assembly seals around the guidewire to prevent air embolism while allowing the operator to advance the guidewire with one hand. The operator's free hand simultaneously manipulates an ultrasound probe or other imaging device, improving speed and accuracy in interventional procedures including central line placement, nephrostomy, and vascular access.
