Disclosed are kits, methods, systems, and devices for reducing mutagen-related biomolecular adduct risk during pharmacotherapy. A therapeutic agent comprising an active pharmaceutical ingredient or a therapeutic supplement is combined with one or more adjunct components selected from probiotics, antioxidants, buffering agents, chelators, plant-derived materials, algae-derived materials, enzymes, vitamins, and combinations thereof to form a mixture that is fermented prior to oral or topical administration. Biomarker data comprising one or more adductomic endpoints is obtained from a subject and compared with functional protein metrics and comparator data, thus subsequent preparation cycles are adjusted by modifying one or more fermentation parameters including duration, temperature, pH, light exposure, concentration, and/or order of adjunct addition. Clinical, pharmacokinetic, pharmacodynamic, and/or functional protein metrics are used to guide adjustment. System embodiments include one or more kits, compositions, vessels, and instructions configured to output updated preparation instructions while maintaining intended therapeutic efficacy.
