Abstract
A method for treating a heart failure patient by ablating a nerve of the splanchnic sympathetic nervous system to increase venous capacitance and reduce pulmonary blood pressure. A method including: inserting a catheter into a vein adjacent the nerve, applying stimulation energy and observing hemodynamic effects, applying ablation energy and observing hemodynamic effects, applying simulation energy after the ablation and observing hemodynamic effects.
Claims21
A method for treating a patient diagnosed with heart failure, comprising:
- advancing through a vascular system of the patient a distal region of an endovascular catheter with retaining a proximal region of the endovascular catheter external to the patient,
- advancing the distal region of the endovascular catheter into an azygos vein or in a T9, T10 or T11 intercostal vein;
- deploying a deployable structure on the distal region in the azygos vein or the T9, T10 or T11 intercostal vein to advance the deployable structure radially outwards towards a wall of the azygos vein or the T9, T10 or T11 intercostal vein;
- transmitting a stimulation signal from the deployable structure;
- determining that the deployable structure is proximate to a greater splanchnic nerve by detecting a response of the greater splanchnic nerve to the stimulation signal;
- after the determination and the detection of the response and while the deployable structure is deployed, activating an energy source external to the patient and in operable communication with an energy delivery element mounted to the deployable structure;
- delivering energy from the energy source through the endovascular catheter to the energy delivery element on the deployable structure while the deployable structure is deployed, wherein the delivering of the energy disables the greater splanchnic nerve proximate the azygos vein or the T9, T10 or T11 intercostal vein;
- and
- removing the endovascular catheter from the patient after the delivering of the energy.

A method for treating a patient including:
- positioning an endovascular catheter comprising a proximal region, a flexible shaft, and a distal region, wherein the flexible shaft connects the proximal region and the distal region, and the flexible shaft has a length sufficient to access a T9, T10, or T11 intercostal vein of the patient relative to an access location on the skin of the patient, wherein the proximal region remains external to the patient, and the distal region includes an electrically conductive element,
- wherein the method comprises:
- advancing the distal region through an azygos vein while the flexible shaft is advanced through a vascular system of the patient;
- advancing the distal region from the azygos vein and into the T9, T10, or T11 intercostal vein;
- transmitting a stimulation signal from the distal region into tissue proximate the T9, T10 or T11 intercostal vein;
- determining that the deployable structure is proximate to a greater splanchnic nerve by detecting a response of the greater splanchnic nerve to the stimulation signal;
- moving the electrically conductive element radially outward towards a vessel wall of the T9, T10 or T11 intercostal vein while the deployable structure is in the T9, T10 or T11 intercostal vein;
- after the determination and the moving of the electrically conductive element, delivering energy from the conductive element to irreversibly disable the greater splanchnic nerve proximate the azygos vein or the T9, T10 or T11 intercostal vein, and
- removing the endovascular catheter from the patient after the delivering the energy.
