A method of assembling a cannulated polyaxial bone anchor includes obtaining a screw shank with an upper capture portion having an internal drive socket entirely surrounded by a planar annular top surface, an anchor portion, and a central bore extending an entire length of the screw shank. The method also includes obtaining a receiver with an open channel above an internal accommodation space having a non-tapered seating surface adjacent a bottom opening, obtaining a compression insert having a lower surface for engaging the upper capture portion and an upper surface for engaging the rod, and positioning the screw shank within the internal accommodation space with the upper capture portion in engagement with the non-tapered seating surface and the compression insert in the open channel with the lower surface in engagement with the upper capture portion except for the planar annular top surface. The method further includes implanting the anchor portion of the cannulated screw shank into the bone of the patient, positioning a rod in the open channel of the with the rod in engagement with the upper surface of the compression insert, and rotatably downwardly positioning a closure top into the open channel to lock the rod to the cannulated polyaxial bone anchor.
A method of assembling a cannulated polyaxial bone anchor into a bone of a patient with tooling and connecting to a rod, the method comprising:
