A surgical patient positioner is described wherein a base plate includes a plurality of tracks configured to slidably couple to anterior and posterior support assemblies, useful for lateral decubitus, hip arthroplasty. The base plate is contoured to fit between the pelvis and rib cage of the patient to avoid pressuring skeletal tissue, and includes ratchet recesses coupled with quick release latches. The anterior tower includes domed pad holders that may couple to the iliac crests. The pad holder supports are slidably adjustable vertically and/or horizontally by means of latching push pins and configured for single hand operation. A lower back support plate is slidably coupled to the posterior support tower and is vertically adjustable.
