Abstract
A system for metatarsal phalangeal joint fusion includes a hinged guide; a trajectory guide attachable to the hinged guide; and an anchoring wire to attach the hinged guide to a bone.

A system for metatarsal phalangeal joint fusion comprising:
- a hinged guide including a metatarsal guide having a first set of trajectory holes and a phalanx guide having a second set of trajectory holes;
- a trajectory guide insertable into one of the first set of trajectory holes and the second set of trajectory holes, the trajectory guide including an upright portion and an indicator portion, the indicator portion including at least two radiopaque indicators that extend parallel to each other;
- and
- at least one anchoring wire configured to attach the hinged guide to a bone.

A system for metatarsal phalangeal joint fusion comprising:
- a first guide including a connecting body, a metatarsal guide, a phalanx guide attached to the connecting body via a lead screw, and a first knob to lock articulation between the connecting body and the metatarsal guide;
- an outrigger assembly attachable to the phalanx guide, the outrigger assembly including an arm anchor and two outrigger arms attached to the arm anchor;
- and
- a pronged trajectory guide insertable into the outrigger assembly, the pronged trajectory guide including at least one radiopaque indicator defining a parallax indicator.

A method of fusing a metatarsal phalangeal joint comprising:
- anchoring a guide to a phalanx and a metatarsal of the metatarsal phalangeal joint using converging anchoring wires;
- distracting the phalanx and the metatarsal via a lead screw;
- preparing the metatarsal phalangeal joint for fusion;
- adjusting dorsiflexion of the metatarsal phalangeal joint and locking the dorsiflexion via a knob;
- compressing the phalanx and the metatarsal via the lead screw;
- attaching an outrigger assembly to the guide;
- selecting a trajectory of a fixation fastener using a pronged trajectory guide inserted into the outrigger assembly;
- and
- inserting the fixation fastener along the selected trajectory.

A method of fusing a metatarsal phalangeal joint comprising:
- placing a guide wire into about a center of a metatarsal head;
- placing a hinged guide over the guide wire through a cannulated hinge of the hinged guide;
- aligning the hinged guide with the metatarsal and a phalanx using radiopaque indicators visible under medical imaging, including verifying alignment with parallax indicators;
- anchoring the hinged guide to the phalanx and the metatarsal using converging anchoring wires;
- distracting the phalanx and the metatarsal;
- preparing the metatarsal phalangeal joint for fusion;
- selecting a dorsiflexion angle and locking the hinged guide to set the dorsiflexion;
- compressing the phalanx and the metatarsal;
- inserting a trajectory guide into a set of trajectory holes in the hinged guide;
- selecting a trajectory of a fixation fastener from among a plurality of trajectory options indicated by the trajectory guide;
- and
- inserting the fixation fastener along the selected trajectory.
