Abstract
Systems and methods are disclosed for providing ongoing monitoring and updating of blood volume status, where the system or method can include guidance in the form of recommendations for treatment actions or alerts about altered patient status.
Claims9
A system for automatically analyzing blood of a living patient and providing euvolemically corrected analyte monitoring, comprising a hematocrit (Hct) monitor;
- an analyte monitor configured to measure a concentration of an analyte in the patient's blood;
- one or more processors operatively coupled to a memory, the Hct monitor, and the analyte monitor;
- a user interface operatively connected to the one or more processors and configured for entry and display of information;
- where the one or more processors are configured to execute programmed instructions stored in the memory to carry out a method comprising the steps of:
- a. receiving data establishing an initial blood volume (BV) and ideal blood volume (iBV) for the patient;
- b. measuring, at one or more later times, an updated value for patient Hematocrit (Hct) from the Hct monitor, and an updated whole blood analyte concentration from the analyte monitor;
- c. calculating, by the one or more processors, an updated blood volume (BV) based on the updated Hct;
- d. calculating, by the one or more processors, a euvolemically corrected analyte concentration by multiplying the measured analyte concentration by a ratio of the updated BV to the iBV;
- and
- e. displaying, by the one or more processors, at the user interface, the euvolemically corrected analyte concentration.
A system for providing treatment guidance for a trauma or critical care patient, comprising:
- a hematocrit monitor;
- a user interface configured for entry and display of information;
- one or more processors operatively coupled to a memory, the hematocrit monitor, and the user interface;
- where the one or more processors are configured to execute programmed instructions stored in the memory to carry out a method comprising the steps of:
- a. receiving data establishing an initial patient total blood volume measurement, an initial patient red blood cell volume measurement, and an ideal blood volume (iBV);
- b. receiving, at one or more later times, an updated value for patient Hematocrit (Hct) measured by the hematocrit monitor;
- c. calculating an updated total blood volume and an updated red blood cell volume based on the updated Hct;
- d. calculating a percentage of total blood volume loss based on a deviation of the updated total blood volume from the ideal blood volume, and calculating a red blood cell volume loss based on a deviation of the updated red blood cell volume from the initial red blood cell volume;
- e. classifying the patient into a specific class of hypovolemic shock based on the calculated percentage of total blood volume loss and the calculated red blood cell volume loss;
- and
- f. generating a treatment alert at the user interface recommending administration of crystalloids, colloids, or administration of blood products, wherein the recommendation is specifically determined by the classified severity of the hypovolemic shock.
A system for continuously or semi-continuously monitoring the hydration status of a living subject, comprising:
- a hematocrit monitor;
- a user interface configured for entry and display of information;
- one or more processors operatively coupled to a memory, the hematocrit monitor, and the user interface;
- where the one or more processors are configured to execute programmed instructions stored in the memory to carry out a method comprising the steps of:
- a. receiving data establishing an initial subject blood volume measurement (initial BV), an initial plasma volume measurement (initial PV), an ideal blood volume (iBV), and an ideal plasma volume (iPV);
- b. receiving, at one or more later times, an updated value for subject Hematocrit (Hct) measured by the hematocrit monitor;
- c. calculating an updated plasma volume (updated PV) and an updated blood volume (updated BV) based on the updated Hct;
- d. determining a hydration status of the subject by comparing the updated PV to the initial PV or the iPV, or by comparing the updated BV to the initial BV or the iBV, wherein the processor uses an increase in the updated Hct as a proxy for a decrease in the updated PV indicating dehydration, and uses a decrease in the updated Hct as a proxy for an increase in the updated PV indicating overhydration;
- and
- e. generating a treatment guidance alert at the user interface recommending fluid intake, fluid restriction, or diuretic administration based on the determined hydration status.
A system for providing treatment guidance for a patient undergoing dialysis or ultrafiltration, comprising:
- a hematocrit monitor;
- a user interface configured for entry and display of information;
- one or more processors operatively coupled to a memory, the hematocrit monitor, and the user interface;
- where the one or more processors are configured to execute programmed instructions stored in the memory to carry out a method comprising the steps of:
- a. receiving data establishing an initial patient blood volume and an ideal blood volume for the patient;
- b. determining an ultrafiltration goal based on a calculated deviation of the initial patient blood volume from the ideal blood volume;
- c. receiving, continuously or semi-continuously during a dialysis or ultrafiltration session, an updated value for patient Hematocrit (Hct) measured by the hematocrit monitor;
- d. calculating an updated absolute plasma volume based on the initial patient blood volume and the updated Hct to monitor an ultrafiltration rate and a plasma refill rate;
- e. detecting, based on a change in the updated Hct and the calculated updated absolute plasma volume, when the plasma refill rate is insufficient to maintain a predetermined minimum plasma volume during the dialysis or ultrafiltration session, wherein the predetermined minimum plasma volume is defined as a function of the ideal blood volume;
- and
- f. generating an automated alert at the user interface recommending an adjustment, tapering, or cessation of the ultrafiltration rate in response to detecting the insufficient plasma refill rate or in response to reaching the ultrafiltration goal.
