An apparatus for performing a medical procedure in a vessel, includes one or more inflatable balloons (14a, b) forming an internal passage for allowing perfusion when inflated, which balloon(s) may be generally annular in shape. A shaft (12) connected to the one or more balloons includes an inflation lumen in communication with the one or more balloons, and a working lumen (26) including an opening for receiving a tool, such as needle (30) for puncturing the vessel, which opening may be between adjacent balloons. Pairs of adjacent balloons may also be provided and connected by a tubular, fluid impervious membrane. A distal portion of the shaft may pass at least partially through a wall of one balloon for allowing the needle to reach an adjacent vessel wall. Related methods are also disclosed.
