Systems and methods for scs therapy optimization

A system, comprising:

• a neuromodulator configured to deliver neuromodulation using one or more electrodes on a lead;
• an accelerometer or gyroscope fitted inside a tip or a body of the lead;
• and
• a processing system configured to:
• receive data from the accelerometer or gyroscope;
• analyze activity, motion and/or posture of a patient from the data received from the accelerometer or gyroscope;
• and
• perform a process, based on the analyzed activity, motion and/or posture, for switching from one program to another program from a set of more than one program programmed in the neuromodulator, wherein performing the process includes automatically implementing the other program or suggesting to switch to the other program.

A system, comprising:

• a neuromodulator configured to deliver neuromodulation;
• and
• a processing system configured to:
• create an ECAP map at an initial programming session for a neuromodulation program;
• before delivering a bolus of neuromodulation therapy, measure a current ECAP map and compare the current ECAP map to the ECAP map created at the initial programming session;
• and
• responsive to the current ECAP map differing from the ECAP map created at the initial programming session by more than a threshold, adjust a stimulation location or prompt a recalibration of the neuromodulation program.
