A device for determining the severity of Freezing Of Gait (FoG) in a patient is provided. The device comprises a FoG registration interface comprising two thumb-actuated buttons for registering the FoG status of the patient as “FoG” (experiencing FoG) or “no FoG” (not experiencing FoG), where one button indicates “FoG” and the other button indicates “no FoG”. While a patient conducts a gait sequence, the device begins by setting the FoG status to “no FoG”; sets the FoG status to “FoG” when a user actuates the “FoG” button; sets the FoG status “no FoG” to when a user actuates the “no FoG” button, and repeats this for each change in the patient's FoG status; and determines the amount of time the FoG status of the patient was registered as FoG as a percentage of total time for the predefined gait sequence.
