Systems, devices, and methods for delivering light to skin with enhanced optical coupling efficiency for photobiomodulation are described. A light emitter is configured to emit light. A conformable index-matching medium, which may comprise silicone and hydrogel with refractive indices matched to skin tissue, is coupled to the light emitter on one side and contacts the skin on another side. Air is removable from between the medium and skin to create an index-matched optical pathway that reduces Fresnel reflection losses. The medium receives light from the light emitter and delivers the light through the index-matched interface to enhance penetration into the skin for photobiomodulation. Structural support elements, cooling systems, air evacuation mechanisms, and controllers may be integrated for optimal device performance.
