An illumination device is provided to incorporate light extraction features in a micro-light emitting diode (microLED). The microLED contains a semiconductor stack that includes n-type and p-type semiconductors, and an active region therebetween. The microLED has an emitting surface from which light generated by the active region exits the microLED. The emitting surface has nanoscale patterned sapphire substrate (nanoPSS) features therein. The nanoPSS features have periodic frustoconical shapes each having a pitch of about 200 nm to about 500 nm, a height of about 200 nm P to about 500 nm, and a spacing of about 20%. Metal sidewalls extend from the emitting surface to form a trench in which the microLED is disposed. The metal sidewalls extend at a trench angle less than about 9% with respect to a normal to the emitting surface and electrically contact the n-type semiconductor.
