An image forming apparatus includes: an image former that forms and outputs an image on a sheet; and a hardware processor that causes the image former to output a sample image on the sheet. The hardware processor changes an image formation condition while the image former is outputting the sample image, based on at least one of sheet characteristic information and environment information.

An image forming apparatus comprising:
- an image former that forms and outputs an image on a sheet;
- and
- a hardware processor that causes the image former to output a sample image on the sheet;
- wherein the hardware processor changes an image formation condition while the image former is outputting the sample image, based on at least one of sheet characteristic information and environment information.
