Abstract
The invention relates to improved culture methods for expanding epithelial stem cells and obtaining organoids, to culture media involved in said methods, and to uses of said organoids.

A composition comprising:
- a. an organoid comprising Lgr5+ epithelial stem cells;
- and
- b. a culture medium comprising a basal medium, one or more receptor tyrosine kinase ligands, a Wnt agonist, a TGF beta inhibitor, and a cAMP pathway activator,
- wherein the Wnt agonist is a Lgr5 agonist, and
- wherein the TGF-beta inhibitor is effective for reducing activity of the TGF-beta signaling pathway that acts via Smad2 and/or Smad3.

A composition comprising:
- an organoid having been cultured in a culture medium comprising:
- a basal medium, one or more receptor tyrosine kinase ligands, a Wnt agonist, a TGF beta inhibitor, and a cAMP pathway activator, wherein the Wnt agonist is a Lgr5 agonist, and wherein the TGF-beta inhibitor is effective for reducing activity of the TGF-beta signaling pathway that acts via Smad2 and/or Smad3;
- and
- the organoid comprises Lgr5+ epithelial stem cells, and expression of Lgr5 in the organoid is upregulated compared to expression of Lgr5 in an organoid having been cultured in the culture medium but without the cAMP pathway activator.
