## Abstract

Methods for off-axis treatment of a vertebral body. A deflectable portion of the steerable instrument is removably disposed within a distal portion of a deformable conduit, both of which are directed to within an access cannula. A control surface of the steering instrument is manipulated with the deflectable portion and the distal portion positioned within the access cannula. The access cannula prevents deflection of the steerable assembly and causes potential energy to be stored within the steerable instrument. The potential energy may be stored as tension on a control element of the steerable instrument. The steerable assembly is advanced relative to the access cannula. The release of the potential energy causes lateral deflection of the deflectable portion of the steering instrument and the distal portion of the deformable conduit to assume a curved configuration within the cancellous bone that is off-axis from a longitudinal axis of the access cannula.

## Claims

### Claim 1

A method of manipulating tissue with a system including an access cannula, a steerable assembly comprising a steerable instrument, a control surface coupled to the steering instrument, and a deformable conduit, the method comprising the steps of:

positioning a distal end of the access cannula within cortical bone to provide access to cancellous bone disposed therein;
directing the deflectable portion of the steering instrument and the distal portion of the deformable conduit to within the access cannula along a longitudinal axis;
manipulating the control surface with the deflectable portion positioned within the access cannula, wherein the access cannula prevents deflection of the steerable assembly and causes potential energy to be stored within the steerable instrument; and
advancing the steerable assembly relative to the access cannula, wherein release of the potential energy causes lateral deflection of the deflectable portion of the steering instrument and the distal portion of the deformable conduit to assume a curved configuration within the cancellous bone that is off-axis from the longitudinal axis of the access cannula.

### Claim 2 (depends on 1)

The method of claim 1, wherein the steerable instrument includes a locking mechanism, the method further comprising stopping the manipulation of the control surface once the locking mechanism retains the potential energy, wherein the locking mechanism is engaged during the step of advancing the steerable assembly relative to the access cannula.

### Claim 3 (depends on 2)

The method of claim 2, further comprising:
actuating a release button to move the steerable assembly to a slack tension mode in which the steerable instrument exerts substantially zero lateral force on the deformable conduit; and
withdrawing the steerable instrument from the deformable conduit, wherein the deformable conduit remains in the curved configuration.

### Claim 4 (depends on 1)

The method of claim 1, further comprising aligning indicia on a proximal section of the deformable conduit to locate a distal end of the deformable conduit relative to a distal end of the access cannula.

### Claim 5 (depends on 1)

The method of claim 1, further comprising observing indicia on one of the deformable conduit or the steerable instrument indicative of a direction of curvature of the curved configuration.

### Claim 6 (depends on 1)

The method of claim 1, further comprising observing visible, tactile, or audible feedback as a desired amount of the potential energy is imparted to the steerable assembly.

### Claim 7 (depends on 1)

The method of claim 1, wherein the step of manipulating the control surface comprises pulling on a trigger towards a steering handle.

### Claim 8 (depends on 1)

The method of claim 1, wherein the control surface is one of a thumbwheel, slider, button, rotatable knob, or combinations thereof.

### Claim 9 (depends on 1)

The method of claim 1, wherein a hub is coupled to the deformable conduit, the method further comprising:
coupling the hub to the access cannula; and
directing an instrument or an implant through the deformable conduit, wherein the hub prevents movement of the deformable conduit relative to the access cannula.

### Claim 10 (depends on 9)

The method of claim 9, further comprising providing an input to an axial controller of the hub to cause axial movement of the deformable conduit.

### Claim 11 (depends on 10)

The method of claim 10, wherein the instrument is an expandable member, and wherein the step of providing the input to the axial controller is configured to retract the deformable conduit and expose the expandable member.

### Claim 12 (depends on 10)

The method of claim 10, wherein the implant is hardenable material, and wherein the step of providing the input to an axial controller is configured to control delivery of the hardenable material at a target site.

### Claim 13 (depends on 1)

The method of claim 1, further comprising observing, under intraoperative imaging, indicia of the deformable conduit indicative of an angle of curvature of the deflectable portion, wherein the indicia comprises at least one of metal reinforcement, filler material in a polymer, or one or more radiopaque markers.

### Claim 14 (depends on 1)

The method of claim 1, positioning a distal end of the deformable conduit at a target site target site that is located in a central location in the cancellous bone of a vertebra that is generally symmetrical with respect to the vertebra.

### Claim 15

A method of manipulating tissue with a system including an access cannula, a steerable assembly comprising a steerable instrument, a control surface coupled to the steering instrument, and a deformable conduit, the method comprising the steps of:

positioning a distal end of the access cannula within cortical bone to provide access to cancellous bone disposed therein;
directing the deflectable portion of the steering instrument and the distal portion of the deformable conduit to within the access cannula along a longitudinal axis;
manipulating the control surface with the deflectable portion positioned within the access cannula, wherein the access cannula prevents deflection of the steerable assembly and causes a control element of the steerable instrument to be tensioned; and
advancing the steerable assembly relative to the access cannula, wherein the tension on the control element causes lateral deflection of the deflectable portion of the steering instrument and the distal portion of the deformable conduit to assume a curved configuration within the cancellous bone that is off-axis from the longitudinal axis of the access cannula.

### Claim 16 (depends on 15)

The method of claim 15, wherein, upon emergence from the distal end of the access cannula, the deflectable portion assumes an angle of curvature associated with an extent of the tension of the control element.
