## Abstract

A medical device for removing a material from a hollow anatomical structure is provided. The device may include a shaft member. The device may include an expandable centering element near the distal end of the device. The device may include a macerator element either attached to the shaft or independent and freely moveable from the shaft. The device may include an aspiration lumen in for removal of material. The device may include a drive shaft attached to a motor and used to rotate the macerator element.

## Claims

### Claim at position 1

1-20. (canceled)

### Claim 21

A system comprising:
an intermediate shaft configured to be coaxially inserted into a lumen of an outer shaft and coaxially over an inner shaft, the intermediate shaft is configured to be uncoupled at a distal end from the outer shaft and the inner shaft, wherein the intermediate shaft is configured to be rotated relative to the outer shaft and the inner shaft;
a helical macerator member at the distal end of the intermediate shaft;
a vacuum configured to be in operative fluid communication with the lumen of the outer shaft, the helical macerator configured to operatively break up at least a portion of an undesirable material within the lumen of the outer shaft upon rotation of the intermediate shaft.

### Claim 22 (depends on 21)

The system of claim 21, wherein the helical macerator member is configured to extend approximately 0.25 inches to 5.0 inches from a distal end of the outer shaft.

### Claim 23 (depends on 21)

The system of claim 21, wherein the inner shaft is a guidewire.

### Claim 24 (depends on 23)

The system of claim 23, wherein rotation of the helical macerator member relative to the outer shaft is configured to pull the portion of the undesirable material within the lumen of the outer shaft.

### Claim 25 (depends on 24)

The system of claim 24, wherein the intermediate shaft is configured to move coaxially relative to the inner shaft.

### Claim 26 (depends on 25)

The system of claim 25, wherein the vacuum source is configured to be activated simultaneously with rotation of the helical macerator member.

### Claim 27 (depends on 26)

The system of claim 26, wherein the helical macerator member is configured to be rotated in a clock-wise direction.

### Claim 28 (depends on 27)

The system of claim 27, wherein the helical macerator member is configured to move coaxially relative to both the inner shaft and the outer shaft.

### Claim 29 (depends on 21)

The system of claim 21, wherein the helical macerator member comprises at least two helical members, and the helical macerator member comprises a pitch in the range of 0.5 helical members per inch to 10 helical members per inch.

### Claim 30 (depends on 21)

The system of claim 21, wherein the helical macerator member is configured to be rotated up to 5,000 rotations per minute.

### Claim 31

A system comprising:
a macerator shaft configured to be coaxially inserted into a lumen of an aspiration cannula and coaxially over a guidewire, wherein a distal end of the macerator shaft is configured to be coaxially movable relative to the aspiration cannula and the guidewire and the macerator shaft is configured to be rotated clock-wise relative to the aspiration cannula and the guidewire;
a helical macerator coupled at the distal end of the macerator shaft;
a vacuum configured to be in operative fluid communication with the lumen of the aspiration cannula, the helical macerator configured to operatively break up at least a portion of an undesirable material within the lumen of the aspiration cannula upon clock-wise rotation of the macerator shaft.

### Claim 32 (depends on 31)

The system of claim 31, wherein the helical macerator is configured to be advanced towards the undesirable material to mechanically disrupt the undesirable material into smaller pieces.

### Claim 33 (depends on 32)

The system of claim 32, wherein the vacuum is configured to aspirate pieces of undesirable material that are broken up by the helical macerator through the lumen of the aspiration cannula.

### Claim 34 (depends on 33)

The system of claim 33, wherein the guidewire is configured to be advanced through the undesirable material to a position distally beyond the undesirable material.

### Claim 35 (depends on 34)

The system of claim 34, wherein the macerator shaft is configured to be advanced and retracted over the guidewire within the lumen of the aspiration cannula.

### Claim 36

A system comprising:
a shaft configured to be coaxially inserted into a lumen of a cannula and coaxially over a guidewire, wherein a distal end of the shaft is configured to be coaxially movable in relation to the cannula and the guidewire and the shaft is configured to be rotated in a clock-wise direction in relation to the cannula and the guidewire;
