Abstract
Embodiments of the present invention address the limitations of current central venous pressure monitoring by providing a noninvasive, non-implanted, and self-administered test for the determination of central venous pressure. Some embodiments use alterations in transmural pressure, optical measurements of venous volume, and anatomical measurements to determine central venous pressure. The system can provide an absolute measurement of central pressure and can be used to monitor relative changes in central venous pressure over time. Changes in transmural pressure are used to create detectable changes in peripheral venous vascular volume for central venous pressure measurement.
Claims14
An apparatus for determining central venous pressure (CVP) of an individual comprising:
- (a) an enclosure having an inside volume and separating the inside volume from the atmosphere external to the enclosure and having an aperture allowing communication between the inside volume and the atmosphere external to the enclosure and configured to receive a terminal aspect of a limb of the individual into the inside volume through the aperture;
- (b) a flow restriction device at the aperture enabling a pressure difference between the inside volume and the atmosphere external to the enclosure;
- (c) a pressure variance mechanism configured to vary transmural pressure on the terminal aspect of the limb within the inside volume;
- (d) a gross volume measurement system responsive to the gross volume of the terminal aspect of the limb and configured to determine a plurality of gross volumes of the terminal aspect of the limb, where each gross volume is determined at a transmural pressure that is different from the transmural pressures at which other gross volume determinations are made;
- (e) a hydrostatic pressure determination system configured to determine a vertical distance between the terminal aspect of the limb and the heart of the individual, and to determine a hydrostatic pressure from the vertical distance;
- and
- (f) a CVP analysis system configured to determine central venous pressure of the individual from the hydrostatic pressure and from the plurality of gross volumes and the corresponding transmural pressures.
A method for determining central venous pressure in an individual, comprising:
- passing a terminal part of a limb of the individual into an enclosure through an aperture and through or into a flow restriction device;
- using the flow restriction device to limit fluid movement between the inside and outside of the enclosure;
- varying the pressure in the enclosure to subject the terminal aspect of the limb to a plurality of transmural pressures;
- determining a gross volume of the limb at each of the plurality of transmural pressures;
- and
- determining a hydrostatic pressure by measuring the height between
- the terminal aspect of the limb and the individual's heart using a fiducial mark or fiducial markers;
- processing the gross volume and the corresponding transmural pressures to determine a peripheral venous pressure;
- and
- subtracting the peripheral venous pressure from the hydrostatic pressure to determine the central venous pressure of the individual.
