Abstract
The disclosure relates to devices and methods for implantation of an orthopedic device between skeletal segments using limited surgical dissection. The implanted devices are used to adjust and maintain the spatial relationship(s) of adjacent bones. Depending on the implant design, the motion between the skeletal segments may be increased, limited, modified, or completely immobilized.

A method for delivery of an implantable device to a target location within a body of a subject, the method comprising:
- affixing a non-implantable first fixation member onto a first bony segment of the subject, the first fixation member comprising a proximal segment and a distal end segment;
- coupling at least a first portion of a platform with the proximal segment of the first fixation member,
- using an imaging modality to define a spatial relationship between at least a second portion of the platform and the target location, the at least second portion of the platform comprising a first bore that extends along a central axis;
- using the defined spatial relationship to move the at least second portion of the platform into a first position wherein, when in the first position, the central axis of the first bore intersects the target location;
- and
- advancing the implantable device through the first bore and onto the target.
A method for delivery of an implantable device to a target location within a body of a subject, the method comprising:
- affixing a non-implantable first fixation member onto a first bony segment of the subject, the first fixation member comprising a proximal segment and a distal end segment;
- coupling a first member of a platform with the proximal segment of the first fixation member, the platform comprising (i) a first member configured to couple with the proximal segment of the non-implantable first fixation member, (ii) a second member configured to couple with an arm member of the platform via a first coupler, the first coupler comprising a proximal first rotational joint configured to provide rotation around a first linear axis, and a distal second rotational joint configured to provide rotation around a second linear axis, the first linear axis non-coplanar with the second linear axis;
- (iii) the arm member, movably coupled to the second member of the platform, and configured to extend from a proximal segment to a distal segment along a third axis;
- using an imaging modality to define a spatial relationship between the arm member and the target location, the distal segment of the arm member comprises an extension member, the extension member comprising a first bore that extends from a proximal opening to a distal opening and along a central axis;
- using the defined spatial relationship to move the arm member into a first position wherein, when in the first position, the central axis of the first bore intersects the target location;
- and
- advancing the implantable device through the first bore and onto the target, the implantable device entering through the proximal opening, advancing through the first bore and exit though the distal opening.
Apparatus for use in delivering an implantable device to a target location within a body of a subject, the apparatus comprising:
- (i) a non-implantable first fixation member comprising a proximal segment and a distal end segment;
- and (ii) a platform comprising a first member configured to couple with the proximal segment of the non-implantable first fixation member, an arm member, and a second member configured to couple with the arm member via at least a proximal first rotational joint configured to provide rotation around a first linear axis, and a distal second rotational joint configured to provide rotation around a second linear axis that is non-coplanar with the first linear axis, the second member movably coupled to the arm member;
- and
- wherein the apparatus is configured for delivering of the implantable device is via at least the method comprising:
- affixing the non-implantable first fixation member onto a first bony segment of the subject;
- coupling the first member of a platform with the proximal segment of the first fixation member;
- determining a spatial relationship between the arm member and the target location;
- using the defined spatial relationship to move the arm member into a first position wherein, when in the first position, a central axis of a bore of at least a portion of the arm member intersects the target location;
- and
- advancing the implantable device through the bore and onto the target.
