Abstract
Methods and apparatus for immobilizing a synovial joint, such as a sacroiliac (SI) joint are disclosed. In one form, a multipiece fixation device, such as a dowel, includes multiple expandable fasteners that are configured to fix adjacent bones of a synovial joint with respect to one another. The expandable fasteners include expansion portions that are expanded radially via insertion of another expandable fastener or an expansion device to fix the expandable fasteners to the bone. The fixation device may be configured to provide for compression or distraction of the bones of the synovial joint while at the same time stabilizing the joint.
Claims34
An assembly for stabilizing a synovial joint, comprising:
- a distal bone anchor having an outer surface configured to fix the distal bone anchor to a first bone of the synovial joint;
- a proximal bone anchor having an outer surface configured to fix the proximal bone anchor to a second bone of the synovial joint;
- a central member disposed between the distal and proximal bone anchors;
- and
- a plurality of fasteners for connecting the distal bone anchor, the central member, and the proximal anchor to one another.
A multipiece connector for stabilizing a synovial joint, comprising:
- a first fastener member having an elongate shaft with an outer surface including a threaded portion thereof at a proximal end of the first fastener member;
- a distal sleeve member having a throughopening extending between distal and proximal ends thereof and an outer surface extending thereabout, wherein the throughopening allows at least a portion of the first fastener member including the proximal end thereof to be inserted into and through the throughopening and extend from the throughopening beyond the proximal end of the distal sleeve member;
- a central sleeve member having an outer surface extending between proximal and distal ends thereof and a receiving portion with an opening at the distal end for receiving at least a portion of the first fastener member including the proximal end thereof;
- a proximal sleeve member having an interior opening and an outer surface extending between proximal and distal ends thereof;
- and
- a fastener sized and configured for being received in the interior opening of the proximal sleeve member to allow the central sleeve member and the proximal sleeve member to be connected to one another.
A multipiece connector for stabilizing a synovial joint, comprising:
- a first member having an elongate configuration with an interior space and an outer surface extending thereabout;
- a second member located proximally of the first member having an elongate configuration with an interior space and an outer surface extending thereabout and having a bone engaging surface portion and an outer sliding portion, the outer sliding portion sized and configured to be received in the interior space of the first member;
- an elongate actuator configured to be received within the interior spaces of the first and second members so that shifting the actuator therein causes the first and second members to be drawn together;
- and
- a first expansion portion extending about the outer sliding portion of the second member outer surface configured to be compressed between the first and second members and expanded outwardly to engage adjacent bone or tissue for fixing the connector thereto when the first and second members are drawn together by the actuator.
A method for stabilizing a synovial joint, comprising:
- forming a recess in a first bone of a synovial joint;
- forming a recess in a second bone of the synovial joint, wherein the recess in the second bone is smaller than the recess in the first bone;
- inserting at least a portion of a first expandable fastener member within the recess of the first bone;
- inserting at least a portion of a second expandable fastener member within the recess of the second bone;
- inserting an expansion member within at least one of the first and second expandable fastener members;
- expanding the first expandable fastener member to fix the first expandable fastener member to the first bone;
- and
- expanding the second expandable fastener member to fix the second expandable fastener member to the second bone;
- wherein expanding the first expandable fastener member includes inserting the second expandable fastener member into the first expandable fastener member, wherein the second expandable fastener member includes a distal end and a proximal end and a trailing proximal portion closer to the proximal end than the distal end that is enlarged relative to a leading distal portion closer to the distal end than the proximal end, and the first expandable member is expanded via engagement with the trailing proximal portion of the second expandable fastener member when the second expandable fastener member is inserted into the first expandable fastener member and after the distal portion passes through the first expandable fastener member.
