(a) administering about 100 to about 480 mg verapamil daily to the patient; and
(b) administering a daily dose of 2.5 mg to 4 mg of rivaroxaban to the patient;
wherein the patient has a creatinine clearance (CLCr) of >50 mL/min to 79 mL/min.

### Claim 2 (depends on 1)

The method of claim 1, wherein a daily dose of 2.5 mg of rivaroxaban is administered.

### Claim 3 (depends on 1)

The method of claim 1, wherein a daily dose of 3-4 mg of rivaroxaban is administered.

### Claim 4 (depends on 1)

The method of claim 1, wherein a daily dose of 4 mg of rivaroxaban is administered.

### Claim 5 (depends on 2)

The method of claim 2, wherein the rivaroxaban is administered twice daily.

### Claim 6 (depends on 3)

The method of claim 3, wherein the rivaroxaban is administered twice daily.

### Claim 7 (depends on 4)

The method of claim 4, wherein the rivaroxaban is administered twice daily.
