Method and system for informed consent

A method for obtaining and documenting informed consent for a procedure from an individual, the method comprising;

• providing a software application on a first computing device for executing on the computing device wherein the software application provides a user interface for obtaining video evidencing informed consent for the procedure, administering a quiz to the individual, presenting a document for signature to the individual, receiving a signature for the document from the individual;
• providing a software application to a second computing device for executing on the computing device when the software application provides a user interface for obtaining video telephony between a health care provider and the individual for audio and video communication between the health care provider and the individual;
• receiving a selection of a procedure for which informed consent is desired into the software application executing on the first computing device;
• capturing video evidencing one or more of the video conference between health care provider and the individual and the informed consent for the procedure using the software application executing on the first and second computing devices;
• presenting the document for signature to the individual using the software application executing on the first computing device, the document indicative of the informed consent for the procedure;
• receiving the signature on the document from the individual using the software application executing on the first computing device, and sending it to a server from the first computing device. The video, the document with the signature, and storing the video in a non-transitory computer readable status medium associated with the server to provide for documenting the informed consent of the individual for the procedure.

A method for obtaining and documenting informed consent for a procedure from an individual, the method comprising:

• providing a software application to a computing device for executing on the computing device wherein the software application provides a user interface for obtaining video evidencing informed consent for the procedure, administering a quiz to the individual, presenting a document for signature to the individual, receiving a signature for the document from the individual;
• receiving a selection of a procedure for which informed consent is desired into the software application executing on the computing device;
• displaying an educational video to the individual using the software application executing on the computing device, the educational video containing educational content about the procedure for which the informed consent is desired;
• capturing the video evidencing the informed consent for the procedure using the software application executing on the computing device;
• presenting the document for signature to the individual using the software application executing on the computing device, the documenting indicative of the informed consent for the procedure;
• receiving the signature on the document from the individual using the software application executing on the computing device;
• and
• sending to a server from the computing device the video, the document with the signature, and storing the video in a non-transitory computer readable data storage medium associated with the server to thereby provide for documenting the informed consent of the individual for the procedure.
