System using patient monitoring devices with unique patient ID's and a telemetry system

A system for using telemetry data based on a patient habit information or patient monitoring, comprising:

• one or more patient monitoring devices that has a unique patient ID, the one or more monitoring devices acquiring patient information selected from of at least one of, a patient's activities, behaviors and habit information, and patient monitoring;
• ID circuitry at the patient monitoring device, the ID circuitry including ID storage, a communication system that reads and transmits the unique ID from an ID storage, a power source and a pathway system to route signals through the circuitry;
• and
• a telemetry system in communication with the patient monitoring device, the telemetry system including a database of patient ID's, the telemetry system in communication with the patient monitoring device and the one or more systems, the telemetry analyzing telemetry data based on at least one of, patient's activities, behaviors and habit information, patient condition, and patient parameter, and creating personalized information about the patient.
