Methods are provided for protecting against ventilation-induced lung injury both directly, by lowering surface tension, and indirectly, by promoting equitable liquid distribution in pulmonary alveolar edema, in which liquid- and air-filled alveoli are normally interspersed. Since a pressure barrier is responsible for trapping liquid in discrete edematous alveoli and the magnitude of the barrier is proportional to surface tension at the air-liquid interface, the present invention provides various means for promoting equitable redistribution of edema liquid amongst alveoli to help protect the lung during ventilation, including: i) use of an additive that lowers surface tension; ii) use of active, accelerated deflation during mechanical ventilation; and iii) high frequency (>50 Hz) vibration of the lung.
A method for promoting equitable distribution of liquid amongst pulmonary alveoli in the presence of alveolar edema with a ventilation means including a conduit for fluidly connecting a source of ventilation gas to a lung, the conduit having a receiving inlet for receiving ventilation gas from a source of ventilation gas and a discharge outlet for discharging ventilation gas to the lung, a vacuum outlet for fluidly connecting the conduit to a source of vacuum pressure, a normally-open proportional valve between the receiving inlet and the vacuum outlet, a normally-closed proportional valve at the vacuum outlet, and a pressure transducer for indicating the pressure within the conduit, the pressure transducer being located between the normally-open proportional valve and the discharge outlet of the conduit, said method including the steps of:
An apparatus for promoting equitable distribution of liquid amongst pulmonary alveoli in the presence of alveolar edema, comprising:
