Abstract
Devices and methods for treatment of prolapsed hemorrhoidal arteries is disclosed. The devices can identify the hemorrhoid and ligate the artery without causing significant pain or distension of the rectum. The artery can be identified with ultrasound. The ligation can be performed using energy and/or mechanical structures, such as clips or rubber bands.

A method for treating a hemorrhoid comprising:
- locating a hemorrhoidal artery;
- immobilizing the hemorroidal artery;
- and
- automatedly sealing the hemorrhoidal artery.
A method for treating a hemorrhoid comprising:
- locating a hemorrhoidal artery;
- immobilizing the hemorroidal artery;
- and
- automatedly sealing the hemorrhoidal artery;
- wherein automatedly sealing comprises sensing flow in the artery, and triggering the sealing when flow is detected.
A method for treating a hemorrhoid comprising:
- locating a hemorrhoidal artery;
- immobilizing the hemorroidal artery;
- and
- remotely sealing the hemorrhoidal artery.
A method for treating a hemorrhoid comprising:
- applying suction to tissue superficial to a hemorrhoidal artery.
A device for treating hemorrhoidal arteries comprising:
- a distal end comprising a remote locating component;
- a ligating component;
- and a fixation component;
- a handle proximal to the distal end;
- and
- a neck between the handle and the distal end, wherein the neck is non-linear.
A device for treating hemorrhoidal arteries comprising:
- a distal end comprising a remote locating component;
- a ligating component;
- and a fixation component;
- a handle proximal to the distal end;
- and
- a neck between the handle and the distal end, wherein the neck is angularly articulatable.
A device for treating hemorrhoidal arteries comprising:
- a distal end comprising a remote locating component;
- a ligating component;
- and a fixation component;
- wherein the distal end has a distal end diameter less than about 3 cm;
- and
- a stopper proximal to the distal end, wherein the stopper has a stopper diameter greater than about 3 cm.
A device for treating hemorrhoidal arteries comprising:
- an orthogonal grid of ligating-locating pairs, wherein each ligating-locating pair comprises an artery ligator adjacent to or within an artery locator, and wherein the grid comprises two rows and two columns of ligating-locating pairs.
A method of ligating a hemorrhoidal artery comprising:
- inserting a device into the anus, wherein the device comprises a sensing element and a ligating element;
- advancing the device so that the sensing and ligating components are superior to the dentate line;
- ligating the hemorrhoidal artery not using hand suturing;
- and
- done without direct visualization
A method for treating hemorrhoidal arteries comprising:
- concurrently ligating all the hemorrhoidal arteries.
An apparatus for treating hemorrhoids comprising:
- a sensor for locating hemorrhoidal arteries;
- an energy probe for thermally ligating said hemorrhoidal arteries;
- a safety mechanism to position the sensor and the energy probe at least 1 cm above the dentate line;
- a processing unit coupled to the sensor;
- and
- an energy generator coupled to the energy probe.
A method of treating hemorrhoids comprising:
- inserting a treatment device through the anus, wherein the treatment device comprises a sensor and an energy probe;
- positioning the sensor and energy probe to at least 1 cm above the dentate line;
- expanding or articulating the distal end said treatment device to allow treatment device to be in contact with the interior rectal tissue;
- articulating said treatment device until at least one hemorrhoidal artery is located, wherein articulating comprises turning and/or translating;
- and
- ligating said hemorrhoidal artery thermally with the energy probe.
An apparatus for ligating hemorrhoidal arteries for treatment of hemorrhoids comprising:
- a tubular housing;
- a suction chamber at the distal end of the tubular housing defining an open cavity for creating a tissue fold through suction, wherein the suction chamber having means for receiving compression clips deployable to at least partially pinch the base of said tissue fold;
- a Doppler ultrasound sensor proximal to the suction chamber and the clip deployment mechanism;
- a handle at the proximal end of the tubular housing with mechanisms to activate suction in the suction chamber and to deploy clips to at least partially pinch the base of said tissue fold.
A method for ligating hemorrhoidal arteries for treatment of hemorrhoids, comprising:
- placing a Doppler ultrasound sensor on a tissue surface of a tissue;
- moving said ultrasound sensor along the tissue surface until an artery is located;
- pinching the tissue perpendicularly to create a tissue fold that disrupts blood flow through said detected artery;
- and
- delivering a compression clip to at least partially pinch the base of said tissue fold to stop blood flow through the detected artery.
