Recording systems and methods efficiently record observations made during a clinical encounter. An exemplary recording system can comprise a graphical user interface having an imaging region, a details region, and an observation region. In the imaging region, the user can choose a body area as an active body area. When an active body area is chosen, the details region can dynamically update to display a set of details that are relevant to the active body area. In the details region, the user can choose a detail as an active detail. The observation region can then dynamically update to include an observation that includes the active body area being modified by the active detail. Accordingly, the observation region can include various observations made during the clinical encounter. Other embodiments of the recording systems and methods are also disclosed herein.
